TRACKER DETAILS

• PERSONAL DETAILS: list personal allergies, diagnoses, emergency contact and doctor details
• MONTHLY OVERVIEW: Add any important reminders, tasks, meetings, and appointments into the relevant boxes. List any reminders and priorities that you would like to address during the month.
• DAILY HEALTH TRACKER: Note down any daily appointments. List any symptoms at the times that they occur. Track meals and allergies. Track severity levels for pain, stress, fatigue and brain fog. Write down your mood and weight. List your blood pressure (AM/PM) and pulse at the time of the blood pressure measurement. Monitor your water intake and note the duration of time spent sleeping and exercising.
• NOTES SECTION: jot down other ideas in your digital planner. Pages are available in blank, lined and grid format. Duplicate when you need extra pages to make notes.
